Concurrent Enrollment Form

If you are a currently enrolled, degree-seeking, University of Illinois student interested in taking courses at Parkland College, you must follow the procedures detailed below for Concurrent Enrollment.

Complete a Parkland College application as a non-degree seeking student:
You must be admitted to Parkland prior to registration. If you have enrolled in a course previously at Parkland skip to step 2. You do not need to resubmit an application, you only need to submit this completed form and your Academic History.

Contact college office:
Contact your UIUC advisor or appropriate designee* prior to enrolling at Parkland to discuss how the class(es) impact your total credit hour enrollment for the term and how they fit into your degree program.

Access Academic History:
Log in to Student Self-Service to access your Academic History. Parkland College admissions’ office will review your academic history on course prerequisites and to verify you meet eligibility requirements for concurrent enrollment status. You must submit the Academic History as a PDF with this form to register at Parkland.
Complete this concurrent enrollment form:
Complete and submit this form along with your Academic History. A confirmation email will be sent to the email provided once your registration is complete.
Tuition:
All domestic students approved for concurrent enrollment will be charged the in-district Parkland College tuition rate. Tuition must be paid by the posted deadline regardless of payment method.
Applying for financial aid?
Students wishing to apply for financial aid toward Parkland courses MUST also complete the Office of Student Financial Aid Concurrent Enrollment Agreement.

Note: If you are applying for financial aid, the OSFA Concurrent Enrollment Agreement requires an advisor’s/ARO’s/assistant dean’s signature, so you should bring this form with you for your meeting with your college’s designated appropriate point of contact.
Requesting Parkland transcripts:
It is your responsibility to have an official transcript sent from Parkland College to the University of Illinois at Urbana-Champaign within two weeks after completion of the term of enrollment.

Follow the below steps to capture an unofficial transcript from UIUC as a PDF file:

Login to your UIUC student account
From the Main Menu of the Banner Self-Service, click REGISTRATION & RECORDS
Click STUDENT RECORDS AND TRANSCRIPTS
Click VIEW ACADEMIC HISTORY
Select appropriate transcript level (undergraduate, graduate, or all)
CTRL+P to print document
In Print Destination, select SAVE AS PDF, then Save.

Although not required, you may upload an additional document to show prerequisites, high school GPA, or SAT/ACT/AP/IB score reports. I understand and agree it is my responsibility to ensure I satisfy the appropriate prerequisites for the course(s) in which I plan to enroll and to ensure the course(s) transfer to the University of Illinois at Urbana-Champaign towards my degree as I intend.

I am enrolled in at least the same number of credit hours at the University of Illinois as I wish to register at Parkland. (For Summer courses - you only have to have been enrolled in the Spring previous or registered for classes in the upcoming Fall term)

Once enrolled at Parkland, I agree to abide by Parkland College’s admission and registration rules and procedures, including adherence to course drop and refund deadlines, noting that these dates and deadlines may differ from University of Illinois academic dates and deadlines.

I understand that requests to drop classes after the deadline will not be approved.
